Request to Discontinue Special Instruction 7/9/08

Ben exhibits age appropriate play skills and pragmatic language. He uses spontaneous speech appropriately to request and respond to both children and adults. His vocabulary is growing daily. Ben’s articulation is good, as well as his intelligibility in both words and sentences. Ben’s play skills are currently age appropriate as he interacts well at play with other children and engages in simple pretend play with them.

Ben is also receiving Speech therapy. He is working on articulation skills and language. His therapist expressed concern regarding his ability to stay focused during an activity and conversation. This no longer appears to be a problem for Ben. In play with one or more children using Lincoln Logs, horses, wagons and figurines, Ben demonstrated good concentration and age appropriate play. Although children and adults walked through the room in which he played he remained focused and tuned in to the play with his peers. He also showed good focused attention during Storybook reading time.

Ben currently receives Occupational and Physical therapy. Although he appears to be functioning adequately during free play, climbing, running, and during meal time, Ben continues to work on functional needs and weak skills in both these therapies.

Ben has made good progress in Special Education and has achieved his functional goals. Based on these results termination of Special Instruction is recommended at this time.

Speech Update 6/24/08

Ben participatedi n symbolic play (going to the farm). He spontaneously produced 3-word utterances 50% of the time. We worked on increasing his utterances (e.g. "it's _____") and increasing his articulation during one-word productions when labeling. Great session.

PT Update 6/19/08

We worked on running and jumping skills today. Ben is initiating jumping up but gets only on tip-toes when trying to do it but this is a good beginning. Also, he is running faster but still doesn't move arms back and forward. We will keep working on these activities. Take care, Marina

Speech Update 6/19/08

Ben worked on imitating two-word combinations during symbolic play. (e.g. "A car", "the boat", "mommy's car") 20% of the time w/maximal verabal cues. He is increasing his ability to follow clean up directions between each activity, and spontaneous language is increasing to 3-4 words.

OT Update 6/10/08

Speech Update 6/10/08

Ben participated in symbolic play and produced 3-word utterances after my model with minimum verbal cues. He answered "where is" and "what is" questions consistently. He followed 1-step directions with increased accuracy.

P.S. I spoke with Roberta yesterday and discussed strategies/goals for Ben. We will continue to work closely together, and will try to speak to Lisa regarding attending activities he may benefit from.
